Jockey Club Post-diagnostic Support in Dementia Care Programme

Supported by The Hong Kong Jockey Club Charities Trusts, Jockey Club Post-diagnostic Support in Dementia Care Programme (PDSP) collaborates with different district elderly community centres and churches to hold health talks and conduct on-site cognitive tests for the elderly. The programme’s social worker visited Evangelical Free Church of China Yan Fook Church and Heritage Baptist Church on 13 November and 21 November respectively, and held health talks for over 300 participants and provided tips to the elderly on how to keep their brain healthy. Afterwards, we carried out an on-site cognitive test for over 100 elderly by using the EC Screen App to screen the potential Mild Cognitive Impairment or early dementia cases with the hope to diagnose and intervene early.

PDSP provides free educational talks and cognitive tests for NGOs, organizations, and those aged 50 or above to screen and be diagnosed by our collaborated General Practitioners and intervene early in dementia.

If anyone observed that the elderly in the family demonstrated a significant decline in memory and cognitive impairment, you are welcome to contact our programme staff Mr. Chan at 2946 8174 or Ms. Yuen at 2946 8187 to arrange a cognitive assessment via telephone for the people in need.

PDSP provides free training sessions for people aged 50 or above who have been diagnosed with Mild Cognitive Impairment (MCI) and early dementia within one year, and 6–9 months supporting services to their family caregivers as well. In view of the latest development of the novel coronavirus infection, the elderly and their caregivers can choose to receive supportive service via telephone, zoom meeting, or self-learning from our e-platform. Please contact Ms. Yuen at 2946 8187 or Ms. Fan at 2946 8188 for more details.

The training was conducted in a pleasant atmosphere; participants raised questions and responded very enthusiastically. We sincerely hope the practice could help them alleviate their difficulties and pressures. The two pieces of training attracted a total of about 340 participants.
